Do you want to be put on life support if you were in a terminal state?
Would you want to be kept alive with a feeding tube?
Where do you want to die?
Are there rituals you want performed at the end of your life?
According to one survey, nearly 75% of us don't have advance directives (living will or healthcare power of attorney). Even a greater percentage have not had a conversation with their loved ones about their end-of-life wishes.
In this workshop, you will explore important questions about end-of-life decisions and care; script a plan of your wishes, as well as, exploring the process of choosing a healthcare power of attorney who will follow your wishes.
This is an interactive workshop with group discussion, exploration of an end-of-life planning template and one-on-one assistance. Living Will and Healthcare Power of Attorney documents will also be available for completion at future time.
