How far would you go to sustain the life of someone you love, or your own?

When the moment comes, and you’re confronted with the prospect of pulling the plug, do you know how you’ll respond?

This documentary examines the complicated reality of today’s modern, medicalized death, and offers an unusually intimate portrait of patients facing the prospect of dying in ways that they might never have wanted or imagined.

"Good Death"Plan

Scripting End-of-Life Wishes

Tuesday, October 10   6:30pm

Knox Community Hospital Urgent Care

Education Room

1490-C Coshocton  Rd.

Do you want to be put on life support if you were in a terminal state?
Would you want to be kept alive with a feeding tube?
Where do you want to die?
Are there rituals you want performed at the end of your life?

According to one survey, nearly 75% of us don't have advance directives (living will or healthcare power of attorney).  Even a greater percentage have not had a conversation with their loved ones about their end-of-life wishes.

In this workshop, you will explore important questions about end-of-life decisions and care;  script a plan of your wishes, as well as, exploring the process of choosing a healthcare power of attorney who will follow your wishes.

This is an interactive workshop with group discussion, exploration of an end-of-life planning template and one-on-one assistance. Living Will and Healthcare Power of Attorney documents will also be available for completion at future time.
